Returning Student-Athletes need the following forms for participation.
Please print off the forms below, fill them out and bring them to the Athletic Training Offices before the season starts.
FOR ALL RETURNING AND NEW STUDENT-ATHLETES - YOU MUST BRING YOUR INSURANCE CARD OR COPY OF FRONT AND BACK OF YOUR INSURANCE CARD TO YOUR PHYSICAL
