Try Outs
Student-Athletes wishing to try out for a Fresno State Division I team need the following forms for participation. Also, ALL students wishing to try out must show proof of their sickle cell trait status prior to beginning their respective try-out. Please contact your primary care physician for proper testing and/or documentation. THIS IS REQUIRED FOR ALL SPORTS SPONSORED BY FRESNO STATE ATHLETICS.
Please print off the forms below, fill them out and bring them to the Athletic Training Offices before going out for the team.
